Torbay Care Trust commission their clinical and social care support from Devon Partnership Trust.
Devon Partnership Trust
Devon Partnership NHS Trust was established in 2001. They employ around 2,000 staff and also have around 100 staff assigned from Devon County Council and Torbay Unitary Authority, including social workers and support workers. DPT serve a large geographical area with a population of more than 850,000 people. DPT work closely with health and social care providers across the Devon and Torbay and deliver services for:

Older people

People with alcohol and substance misuse issues

People with a learning disability

People with a severe eating disorder who require a stay in hospital

People who need forensic or secure mental health services (usually people that have come into contact with the criminal justice system).

Devon Partnership Trust has agreed it’s objectives for 2009/10 with Torbay Care Trust.

Improving access to psychological (talking) therapies

Involving people in their care and making sure everyone has an up-to-date recovery plan

Delivering more services locally and reducing the need to travel outside the county.
Community based care is commissioned via Devon Partnership Trust and this includes residential care, home based support, and day care.
Other services purchased by Torbay Care trust include relationship counselling and support, and advocacy services. Employment support services are also currently provided via PLUSS.
Learning Disability services
Specialist clinical services are provided by Devon Partnership Trust.(Please see above for information.)
The services Devon partnership Trust provide for people who have a learning disability include:

In-patient units - providing inpatient care that includes short-term assessment and treatment, specialist provision for autism, and longer-term care. These are consultant led.

Specialist learning disability residential and nursing services, and contributing a significant number of staff to some similar services delivered by a partner housing association.

Community team involvement of professions - including nursing, physiotherapy, occupational therapy, speech and language therapy, consultant psychiatry, and psychology.

Some associated services including therapeutic day time services and Additional Support Teams linked to community teams.

In addition a range of social care support is directly purchased by Torbay Care Trust which includes day opportunities, residential support, enhanced domiciliary care, employment support and advocacy support.
We also actively encourage individuals to have individual budgets to plan and pay for their own care and support. This can be arranged to buy both health and social care support in creative ways that meet the outcomes identified by individuals.
